Paciente de 45 años en tratamiento con fianasteride/minoxidil hace 5 años. Antecedentes de grave accidente craneal 15 años atrás, por el cual fue operado en dos ocasiones por neurocirugía dejando múltiples secuelas cicatrizales en frente y cuero cabelludo. Un año antes se realizó cirugía con robot ARTAS (2000 grafts) en clínica nacional con nula supervivencia. Ante los citados antecedentes se decide realizar prueba de supervivencia en la zona frontal para confirmar que los tejidos eran aptos para la correcta supervivencia y que el fallo anterior se debió sólo a causas técnicas. Se injertaron 113 grafts (2,21 pelos/grafts; 0.9 mm punch, 16 singles, 63 dobles, 28 triples, 6 cuádruples), y se esperó un año para ver resultado.
Test al año positivo por lo que se planteó estrategia en 2 cirugías para reparación frontal, cicatrices en zona media y coronilla. En su primera operación nos concentramos en la parte frontal extrayendo 1901 grafts que se injertaron en primera linea (1700 grafts) y 200 grafts más para diferentes cicatrices en la zona media. Tejido muy fibrótico, técnicamente muy difícil por la dureza, con popping/bledding importante sobre todo en ambas entradas, por lo que aún con experiencia la supervivencia es normal que pueda verse comprometida (4606 pelos, 2,42 pelos/graft; 50 micras de pelo; 0.9 mm punch; Grafts: I-225; II-774; III-785; IV-107; V-10).
A los 14 meses se realiza la segunda cirugía para coronilla (700 grafts), cicatrices (150 grafts) y refuerzo de frontal sobre todo entradas donde más débil quedó la cobertura (700 grafts), con un total de 1500 grafts (3578 pelos; 2,38 pelos/grafts; 0.9 mm punch; Grafts: I-190; II-622; III-589; IV-91; V-1). Nuevamente gran dificultad para la implantación en zona fronta y cicatrices con popping/bledding intenso.
Se presenta resultado a los 10 meses de la intervención con mejoría de las áreas tratadas y cobertura adecuada que permite ocultar los estigmas de sus cirugías iniciales.
This 45 year-old patient has been in medical treatment with finasteride/minoxidil the last 5 years. Background of a serious cranial accident 15 years ago, for which he had 2 neurosurgery procedures leaving multiple scar sequelae in forehead and scalp. A year earlier, a hair transplant surgery (2000 grafts with ARTAS) was performed in a national clinic with zero survival. Given the aforementioned background, we decided to perform a survival test in the frontal area to confirm that the tissues were suitable for a correct survival and that the previous failure was due only to technical issues. 113 grafts (2.21 hairs / grafts; 0.9 mm punch, 16 singles, 63 doubles, 28 triples, 6 quadruples) were grafted, and a year was expected to see the result.
With the positive test, we planned a 2 step strategy to repair the frontal area, scars in the middle area and crown. In his first procedure, we focused on the front part, extracting 1901 grafts that were grafted in the first line (1700 grafts) and 200 more for different scars in the middle area. Very fibrotic tissue, technically very difficult due to hardness and lack of elasticity, with important popping and bledding especially in both corners, so that, even with experience, survival could be compromised (4606 hairs, 2.42 hairs/graft; 50 microns of hair; 0.9 mm punch; Grafts: I-225; II-774; III-785; IV-107; V-10).
At 14 months the second surgery was performed for the crown (700 grafts), scars (150 grafts) and frontal reinforcement mainly in the corners where the coverage was weaker (700 grafts), with a total of 1500 grafts (3578 hairs; 2,38 hairs/grafts; 0.9 mm punch; Grafts: I-190; II-622; III-589; IV-91; V-1). Again great difficulty for implantation in the frontal area and scars with intense popping/bledding.
Final outcom is displayed at 10 months after the second procedure with good improvement of the treated areas and an adequate coverage that allows hiding the stigmas of his former surgeries.
