A patient with gout for many years comes in with gout crystals (tophi) breaking through the skin of the big toe. His diabetic neuropathy prevents the severe pain that is usually associated with the condition. Gout arthropathy is due to a high level of uric acid in the blood stream. Over time the uric acid that collects in joints can crystallize, causing an exteremly painful inflammatory reaction. As years go by, tophi, a pasty collection of uric acid crystals can form.
